Dr Costellanos

I have been a patient of Dr Castellanos in Phoenix for several years. I received a ltr from the hospital indicating that Dr Castellanos will be leaving his practice in mid-August. Does anyone know where Dr Costellanos will be going? The practice has also indicated that we can transfer to 2 other pelvic specialists in the office - Dr Afaupe or Dr Gubbels. Has anyone had any experience with Afaupe or Gubbels? BTW, I understand that Hibner, also in PHX is no longer accepting new patients.

Dr C is moving out of state. That is all he let anyone know.

Couldn't find any contact info for Dr. Castellanos in Orlando. If anyone finds out how to contact his new practice, please let us know.

My understanding is that Dr. Gubbels is practicing in NY. Does anyone know if that's still the case?

I heard from someone that she moved to AZ. I googled her and it looks like she is at Dignity Health in AZ now (where Hibner used to be):
