The thing is, the myofasical release brought a lot of relief. Before I did the sphincter massage things were so tight that every bowel movement hurt and standing or walking for long periods would cause me pain. It was so tight that whenever I opened my legs it felt like air was going in me and allowing that area to breathe. I no longer feel like that. But this last bit of tension I can't release, I still can't sit without pain and my bowel movements do not feel natural. I'm going to try a different physio now, just to see if they have different methods.
My current physio is far too gentle and talks too much, I'm not happy with her.
I'll also see what the colorectal surgeon has to say. I want to avoid surgery but if that's what it takes then that's what it takes.
I'm sure as the tension goes away so will the pain. When my bowels are full I can sit with no pain, I just feel a dull muscular ache in my sphincter but none of that sharp tingling burning pain. This means that something needs to be stretched, and I'm sure it's my sphincter muscles. It's likely that I'll have to wait 2-3 weeks before I see the colorectal surgeon so I'll continue the physio in the meantime.

I have a similar problem. Since my rubber band ligation for my internal hemorrhoids, I am having the rectal pain, spasms and a descending perenium, I was also told about my anal fissure but again I am scared to do the surgery for fissures because I feel my doctor has damaged my rectal branch of the pudendal nerve and doing that surgery will not improve my symptoms.

I am wodering also about the chicken and egg thing but it may stay a mystery. I had pelvic muscle spasms twice that I got cleared up and was pain free before the third round and PNE. I do feel it was probably the start of my PNE. The third time the botox cleared them up but I still had pain from PNE, the cause at that point was sitting on a hard chair in pain and not getting up. With sitting many hours to avoid pain in my feet this was just too much. I wish you the best and always hope for others that it is just an isolated muscle spasm.
2007-08 pelvic muscles spasms treated by EGS. 6/27/10 sat too long on hard chair- spasms, EGS not work Botox help, cortisone shots in coccyx help, still pain, PT found PNE & sent me to Dr Marvel nerve blocks & MRN, TG left surgery 5/9/11. I have chronic bunion pain surgery at age 21. TG gave me back enough sitting to keep my job & join in some social activities. I wish the best to everyone! 2019 luck with orthotics from pedorthist & great PT allowing me to get off oxycodone.

Even when I pass wind I feel the muscle spasm. It's like my body gets ready and then my rectal muscles stop midway through the motion and then finally it comes out.

I'm seeing a different physiotherapist tomorrow. I really hope she's better than the last one. The last one has refused to do biofeedback and spends most of the time talking.
I'm also waiting to get referred to a colorectal surgeon.

I've been doing a pelvic floor stretch for a couple of days and it seems to be helping. It's like a squat but lying down. I would do it squatting but i don't have the flexibility.

My bowel movements now feel more normal. Passing gas is also easier.
I had lost the ability to pass gas silently, this must be because of the tightness of the sphincter. But this is also returning slowly.

My physio was far too conservative, she said to hold the stretch for 30s and do four repititions, and do this three times a day. I'm holding it for 60-90s and doing at least 6 repititions, 6-7 times a day. Only now am I seeing some benefits.

I lie flat on my back and grab my ankles. Your knees should be pointing outwards. While holding this position, breathe through your stomach and push your pelvic floor outwards.

I actually tried it in the standing squat position today while holding onto a chair and got a more deeper stretch, so I'll do it that way from now on.

One thing I have noticed is that when I sit down, as soon as the pain starts the tightness goes away and having bowel movements and passing gas become easier. It's strange. When the muscle is in pain it stops spasming.

Does anyone know how to stretch your sphincter at home? I think stretching the sphincter will stop my problem.

Or is this better left to an expert like a colorectal surgeon.
