Can Alcock's Canal be properly decompressed laparoscopically?

I am looking to possibly have my pudendal nerve decompressed by Dr. Bollens, as it seems that I may have compression in the ischial spine/Alcock's Canal region.

He performs surgery laparoscopically.

However I am not convinced that is the best approach for Alcock's Canal. I know other surgeons do an "open surgery" and decompress the Alcocks Canal region under a microscope, as I believe the nerve isn't visible to the naked eye at this level.

Does anyone have any info on one method being better than the other for this area?

I was directly told by more than 1 laparoscopic surgeon that they cannot decompress the alcocks canal via this method. They said it has to be done via transgluteal surgery.
May 2002 - developed disabling PN.

Aug 2002 - started 4 rounds of CT scan guided nerve blocks

Feb 2003 - had bilateral decompression surgery. The surgeon found a very obvious abnormal issue on the left side that was wrapped around my nerve and squeezing it flat. Slow improvement over the next 6 years.

6 years after surgery Egoscue Method helped me completely get rid of lingering symptoms.

No issues with PN for 14 years.

Dr. Bollens laparoscopic approach allows access to the pudendal nerve from the ischial spine into Alcock’s canal, and has several advantages.
It’s minimally invasive, avoids cutting through muscle, allows for bilateral decompression in one session, and offers a quicker recovery compared to open surgery. While some surgeons use open microsurgical techniques for very distal cases, studies and surgical experience show that the laparoscopic method provides excellent visibility and effective decompression for most entrapments in the ischial spine/Alcock’s canal region. For many, it’s a highly effective and less traumatic option.

Yes, it does. I would ask the surgeons you consult with; I consulted with several of them and the laproscopic surgeons universally told me yes.

Yes, of course! I had laproscopic surgery with Dr Bautrant and his surgical report indicated Alcocks canal was fully dialated and decompressed during surgery. I cannot recall his name, but the second was the surgeon in Portland Oregon that works at the Pearl Womens Clinic, it could be Dr Fogellson and also a Dr Grubbells who all do laproscopic. I went with Dr Bautrant because he has done more than a thousand of these surgeries. I hope they can be of help to you.

I tried contacting Dr Bautrant's office in 2 or 3 different ways earlier this year and couldn't get a reply from them. What contact information did you use to successfully get a consultation and schedule surgery? And do you speak French? (I wondered if my lack of knowing French could have had an impact on them ignoring me)

Sometimes he's on travel so he doesn't answer right away. He speaks English very well.

PNE since 2002. Started from weightlifting. PNE surgery from Dr. Bautrant, Oct 2004. Pain now is usually a 0 and I can sit for hours on certain chairs. No longer take medication for PNE. Can work full time and do "The Firm" exercise program. 99% cured from PGAD. PNE surgery was right for me but it might not be for you. Do your research.

Thank you so much! I've read online that he only does in person consultations rather than doing telemed consultations. Do you know if this is the case? I had transgluteal decompression surgery done 20 years ago and the issue I've been encountering is that most surgeons don't want to do "redo" surgeries so I would certainly want to know if he's open to doing it before flying to France.
