Those of you using a Valium cream can you tell me how the script reads?

I haven't used it but there is a pharmacy website that shows you what they require for their scripts. Looks like you can get 2 different kinds of valium (diazepam) cream - one rectal and one vaginal. If you go to each of the links below and scroll down to where there is a blue click button that says "print or save as pdf" if you click on that blue button a form will come up that shows you how the prescription should be written in the Prescription Details section.
