Some antibiotics have anti-inflammatory properties. See this publication:

Abstract

Although the empirical use of some antibacterial agents in various inflammatory diseases has proved beneficial in the past, it is only recently that the therapeutic anti-inflammatory potential of cyclines and macrolides has received attention worldwide. Other drugs like fosfomycin and some quinolones are also contenders for this role. Owing to the complexity of the inflammatory process, which involves many redundant cellular effectors and soluble mediators, and the obscure etiopathogenesis of many inflammatory diseases (some of which may be secondary to persistent infections), the precise mechanisms underlying the therapeutic benefit of anti-inflammatory antibiotics remain to be elucidated. Structure-activity relationships are the key to developing new anti-inflammatory agents derived from antibacterial drugs.

Since the SI joint seems to be a huge cause of PN according to Peter Dornan, is there a PT in the USA esp on the east coast who is as PN knowledgeable as Peter Dornan?
I don't think I have SI joint issues based on all the opinions I ve gotten, yet I have anal pain that is indescribabe and crippling.
My friend has SI Joint pain and she pointed out the spot where is located..She got a cortisone shot is and is fine...
Not a spot where I have pain..but who knows?

I have seen a zillion docs and several PTs at the various stages of my pain issues over the 6 years of my pain condition.
I have never gotten a true root cause of my pain from a PT.
I remember seeing PTs in my home state (NJ) at the start of my sitting pain in 2009, hoping to nip it in the bud.
They examined me, did the typical internal and external stuff, and all they could come up with at the time is "your butt hurts cause its too skinny and has no padding".. (I had lost weight due to the very limiting interstitial cystitis diet and lots of worry over my pain and of what I could eat etc) What a frustrating answer!

As time went by I saw 2 well known expert PTs in San Fran in 2009, still didn't get any definitive info on what caused my pain.. after traveling across the country. These gals were considered the gurus of pelvic pain PT??
Time went on and in the past year Ive seen one of the best and well known PTs in the NY area and maybe even the country.. I like her and she has been quite supportive.
She says I am tight in various areas but after a few mos of work I didn't feel better.
She hasn't really mentioned the SI joint with regards to my pain but I will ask her about it,as she is very responsive.. I have taken a break from PT for now.
My long term pelvic pain doc never mentioned it ...now she is retired from practice... so I will run this by her colleague who is now my new doc.
Should I be trying to ensure that the SI joint isn't the culprit here...just dont want to ovelook a "missing piece"
Would appreciate any info
Thanks so much!

Hi kathyd, Apparently Peter Dornan is supervising two PTs doing Phd PN research on the West Coast of the States.
Yeah, this is all truly complex in many cases. After reading posts on this site for a while it seems some PN/PNE responds to conservative treatments, and even surgery of various kinds, but for others there seems to be more complex issues at play.

Wishing you all the best in your search for a safe and effective treatment.

Cheers

Kathy, Jerry Hesch who is a manual therapist might be able to give you the name of someone on the east coast. He's in Vegas but I've heard he may be able to give you the names of other practitioners around the country. http://heschinstitute.org/
