Natasha, I'm sorry it is so difficult for your mom to find good health care. Some people can manage their symptoms just with medications and lifestyle changes. Other people have good results with physical therapy or pudendal nerve blocks. Some people don't have success with those types of conservative therapies. If they are in a lot of pain and nothing else helps and they have a diagnosis of pudendal neuralgia, they may choose to try pudendal nerve decompression surgery.
Unfortunately, I don't think a western doctor would travel to the Ukraine -- the Ukrainian doctor would have to travel to learn the procedure. The French doctors were some of the first to perform PNE surgery. Doctors from around the world traveled to France to learn the procedure. If there are no Ukrainian doctors who are willing to learn about PN, then your mom will have to either be satisfied with medication or she may have to travel. Many of us have had to travel even through we were in terrible pain. It was the only choice we had.
I have never heard of gabapentin losing its effectiveness. It is also an anti-seizure drug so you should not go off of it suddenly. You have to go off slowly or you could have seizures, so she needs to be sure to take it as prescribed.

Please help - possible PNE
Re: Please help - possible PNE
PNE since 2002. Started from weightlifting. PNE surgery from Dr. Bautrant, Oct 2004. Pain now is usually a 0 and I can sit for hours on certain chairs. No longer take medication for PNE. Can work full time and do "The Firm" exercise program. 99% cured from PGAD. PNE surgery was right for me but it might not be for you. Do your research.
