I appreciate this can be a difficult decision, especially if you don't have all the typical symptoms. There isn't a definitive test but have you considered nerve conduction studies or seeing a neurologist? What did Hibner say to that? Have urology ruled out urologic causes? I am in the UK but I have all the typical symptoms of PN, I am awaiting nerve conduction studies. I am happy to have surgery without nerve conduction studies but at my surgeons request I will have them.
Neuromodulation requires invasive surgery though and there can be quite a lot of hassle with it and potential complications. Have you spoken to anyone about neuromodulation or explored whether it would even work for you? I understand for some, they do not get any or limited benefit.

I did those tests , they can be perfect normal and you can have entrapment.they have zero value.Sadly i know to much about this problems and i met some of best doctors in world.I spend 30k + euros already......I saw best urologist , therapeuts , surgeons etc.I simply don t know why they want that stupid test in your case.I met a person with a real entrapment that did 2 test 1 normal and 1 abnormal......I try to solve my problem with Dr De Watcher but i still study myself all options since is best to do your own research also.Imo neuromodulation is way less invasive than surgery .....and it can be removed , surgery it can t.And i am a unlucly person in general but i won t say no to it if signals are very clear i should do it .
Also i simply don t know what doctor except Bautrain do surgery on males and can remove entrapment on perineal branch.Beco was very good dr Itza told me but i heard he don t operates males anymore.....Bollens say now he also can decompress those branches but he is a laparoscopic specialist from abdomen in general so his experience with distal branches is small....I have a video call with him and i dislike him.He push you to surgery instantly....he don t like informed patiens , he deleted some of my questions from his youtube videos etc.I don t trust Bollens at all ....also he tripled his surgery price or even more in last 2 , 3 years.......I am very interested in surgeons except Bautrain that can decompress perineal branch in case i will need for future.So if any knows names please let me know.Also the fact Bautrain can see ppl in 1 year or more for a simple consulation is a big red flag for me......

I think the nerve conduction studies are to justify the surgey. I am aware that the French no longer use them, as you can get false negatives. The problem is because there isn't a consesus of opinion, individuals do different things. Personally, I am very happy to just have the surgery...

PNE since 2002. Started from weightlifting. PNE surgery from Dr. Bautrant, Oct 2004. Pain now is usually a 0 and I can sit for hours on certain chairs. No longer take medication for PNE. Can work full time and do "The Firm" exercise program. 99% cured from PGAD. PNE surgery was right for me but it might not be for you. Do your research.
