Tried a PN nerve block last week. National spine and pain in Reston VA does them but only at ischial spine. Almost went to UMD for a fancier block but they said it’s a 6 hour day and you need a complete physical to do it! As usual with any nerve blocks I have tried over 8 years it caused a flare up and did nothing good. I did a 6 day prednisone course for the flare. By day two all my pain was gone. But a day after finishing prednisone it came back. I’ve had same results from oral steroid once before. Almost suggests it’s all inflammation. Tried Celebrex , a different anti inflammatory, for 15 days once and did nothing. Wish there was another type of anti inflammatory

Interesting. When I was in the very early stages of this (before I was diagnosed), a doctor prescribed an oral steroid---I think she thought it was something else and wanted to try this as a solution. And I do remember that that reduced my pain, which made me convinced that she had solved it. But, of course, it came back again. Yeah, it does seem like inflammation is involved. The radiologist who reviewed my MRI (years later) identified edema in the area between the SS and ST ligaments on the side where I had my pn pain, so that suggests the entrapment creates inflammation (or, if it is not entrapped, maybe other sources of inflammation create the pain by squeezing the nerve?).
