I am a 54 year old male who experienced a car wreck on January 19 of this year. I was hit from behind by a pickup truck while waiting for someone in front of me to turn tight. My car was totaled and I had a concussion as my head slammed back into the head rest. It was a serious jolt as my car missed the car in front of me and I came to a stop approximately 50 yards down the road.

The next day I went to my gp with bad head ache and very sore neck. He told me to give it a week to see if my concussion symptoms stopped. For the next three days I continued to have a headache and sore neck with no other symptoms... then on the 4th day something changed.

I started having tremendous pain in my lower back, pelvic region and sciatica in both legs... primarily the left. By that afternoon which was a friday I called my gp and told them of my pain onslaught and that I needed something immediately to get me through the weekend. They told me to take some advil and if it didn't help to call them Monday. First thing Monday I was at their office.

As my concussion symptoms had subsided they prescribed thirty days of hydrocordone and flexorill. In addition to the pain over the next few gays it became very difficult to sit... I had pain in my lower back as well as pain in my pelvic region... for the following two or three weeks I was in bed.

Without getting better I waited until one month from the accident date and went back to my gp. They did an xray and found that I had a grade 1 spondlyliosithesis at the L5 S1 vertebrae with bilateral pars defects. Degenerative disk disease. I was referred to a neurologist as by now my left foot was getting numb. Later that week I saw the neurologist and he ordered an mri of the L5 S1 area which confirmed the xrays and further showed the severe stenosis in my right foraminal at the nerve root... mild stenosis on the fight side.

He ordered an immediate nerve block at the L5 S1 which was completed and luckily stopped the numbness in my left foot but did nothing for any of the associated pain. I convinced him to give me another thirty days of pain meds also. I kept telling him that something else was going on... pain in my groin area and butt was becoming more severe than my L5 S1. He ordered the second block in the same area. Leading up to the block I would describe my pain when sitting was basically like a wire from the left side of my anus... going up the left side of my rectum underneath my sacrum and into the left side of the L5 S1. I told my block doctor this so he gave me the second shot slightly lower than the first... still on the left side. This shot almost eilminated the pain coming out of my left side at the L5 S1... but id did nothing for the wire pain in my rectum or adjacent pelvic regions. My neurologist referred me to a neurosurgeon for a second opinion. I described my symptoms but all the neurosurgeon seemed to care about was the mri... he said I needed spinal fusion.

By now it had been over three months since the accident and I was starting to see the pattern. I could drive for an hour or so with the heated seats on high... I could not sit anywhere else... at my desk or anywhere. If I did too much on a particular day then the next three days I was basically incapacitated. I forgot to mention that I started pt after the first block. The stretching and exercise for my sciatica and lower back have evidently not helped my pelvic area as I have progressively gotten worse. The pt also told me that my sacrum was rotated and my left leg was longer than my right.

To top it all off last week I had blood in my semen. Negative for infection but they put me on three weeks of cipro anyway. The prostate exam was not fun for my rectum. Now I can hardly get out of bed with out an electric shock like pain in my left perrinium just above my anus. My hips are killing me and I am cramping from my hips to pelvis... primarily left.

The one thing that I left off that appears to be different from other cases is that along with my constipation I have a pressure type pain behind my sacrum and coccyx. The difference is that as I am passing a stool the pain is completely gone... for five seconds after too. The same thing even when I pass gas... no pain for five seconds.

Now I have to tell my neurologist, neurosurgeon and urologist that I have this imaginary condition and I need to convince them that I need my third epidural block at my pudendal nerve... and of course the cipro isn't working.

Thanks for the site. Any insights would be appreciated.

Cody

Welcome to the forum Cody,
So sorry to hear you are having so many pains from the accident. A few things I noticed in your write up are the electric shooting pain whuch is anerve pain symptom. Also the rotated sacrum and shorter leg with painfull hips may be signs of rotated pelvis. Some ideas are to see the FAQs off the home page that include a letter to medical professionals that may help with explaining this condition. There is a list of PN - aware Physical therapists off the hiome page that may be better able to address the PN issues. Good luck. There are a variety of treatments (shockwave to mind body, etc to try or review to see what may help).

Hi Cody,

There's a lot of complex anatomy in the pelvis and it can be really difficult to determine the source of pelvic pain. Hope you've had a chance to look at the anatomy page on the website. http://www.pudendalhope.info/node/13 There is a peripheral pudendal nerve on each side of the body with three branches of the nerve on each side.

The inferior rectal nerve, a branch of the pudendal nerve, innervates the anal sphincter; and the anatomy of that nerve varies from person to person. I don't know if that branch of the nerve is one of the sources of irritation for you but it could be. If you could see a PT who specializes in pudendal neuralgia they might be able to give you some additional information to go off of.

You might want to read the final question in a recent interview with Dr. Attaman where he discusses how to tell if the problem originates from the spinal area or from the peripheral pudendal nerve. You can access the interview from the homepage at pudendalhope.org. Scroll down to the end of that interview for the final question.

Just be careful not to rush into any type of surgery without trying some of the conservative treatments first because surgery is not always the answer and has its own set of risks.

Take care,

Violet

Thanks Violet. I hope to convince one of my docs to do another mri at the sacrum nerve roots and lower. I had no pain with direct pressure on the sacrum but continue to have a dull pain underneath my sacrum when I lie down on my back.

Well, I hope you can get this sorted out, Cody. The accident sure messed things up for you and I'm sorry it's been such a struggle.

Take care,

Violet
