My daughter is 11 & has had Pudendal Nrv Entrpmnt for 6 yrs!

Post by Tammy43 »

Thank God I found your site!
It has has been such a long road!
I am going to try to make this short...I wish myself luck! :)
Our daughter is 11 yrs old and suffers daily from what we now know as Pudendal Nerve Entrapment. We were and still are at our wits end.
When our daughter first started having symptoms, we knew it as a very sore, red vulva. She described this as “like boiling water was poured on me”! She was in extreme pain! We didn’t realize that the pain came all the time...not only when the vulva was red and inflamed. We thought that the pain was a symptom of her Vulva getting the “burn” or just recovering from the “burn”. We didn't understand....she wasn't red....why is she in pain???? It took us a long time to "understand" the pain. When nerves are inflamed the inflammation can come out of the skin (redness). The "hot boiling water" or "acid like burn" is not only “on” the skin. It happens on a daily basis “inside” and “out” without the redness.
A little girl only explains it as well as she can and the parents always want to believe their child, but not knowing...what do you do? When a young girl has these types of symptoms, before examination, some of the first thoughts are abuse. This in itself is extremely hard for my husband and I. After examining, it could always be seen that it was not. Every time we see a new Doctor the question is still there. It is heart breaking!
Through the years she has been sent to her Pediatrician, a Dermatologist, 3 "adult" GYN's, had Acupuncture, Magnetic Stimulation, seen a Chiropractor, seen an Allergy specialist, a Physical Therapist, tried pain medications, had and MRI, 3 Ultrasounds, tried WAY TOO MANY creams and the list goes on! She has been checked for everything that you could imagine. Nobody was able to diagnose our daughter.

Talking about it to family, co-workers and friends, researching on-line and finally telling the Doctors what WE want, we found out what she has! It took way too long! :( Sadly this is not an easy subject/medical condition to talk about (because of the area). For an adult and for a child, (now young girl). It is hard to say..." I have chronic pain. I have Pudental Nerve Entrapment." "What is that?" "It is a burning, loss of sensation or numbness, increased sensitivity, electric shock or stabbing pain, knife-like or aching pain, feeling of a lump or foreign body, twisting or pinching, abnormal temperature sensations, constipation, pain and straining with bowel movements, straining or burning." "Where does it hurt?" "My private parts, my back and my thighs." Yeah, o.k., that goes over well with people that you don't know well or my daughters friends. So we keep a lot in. Only "tell" when necessary. :( Although, I have to say we do talk about it to more people then we use to ....for our daughters sake! Most of the time we say she has Puental Nerve Entrapment and they don't ask anything further....:). I, at times have tried to explain it in a little more detail. Again, once you mention anywhere near "that" area I feel that it is their first assumption.....abuse. :(
To top things off, our daughter is also suffering from NO SLEEP! She cannot sleep, she has a hard time falling asleep and then when she finally does, she wakes up 3-4 times a night! I have to sleep with her every night.

Everyday is a tearful day for her. Her pain is mentally and physically exhausting! The pain consists of needle like pains to knife stabbing pains. It can be an awful, big, painful constant pinch. Sometimes it feel like its an electrical shock and sometimes its throbbing, pulling or tugging. The pain will stop her in her tracks. Her clothing consists mostly of yoga pants and sweat pants.
The episodes of pain are not consistent. They can last from 5 minutes, 1hour, 1 day or several days! Sometimes they will come one right after another and sometimes she gets a break in between. She does not get the “burn” as much anymore, but she does get it. Everyday we wait one more day until her next appointment. With the pain comes "depression" (I hate that word!). In the past years and only a couple of days ago she has said things like... "Mom, The pain is so bad, sometimes I want to kill myself." "I would be better off in heaven with Meme (grandmother). At least I wouldn't have any pain there." "I wish I could cut my private parts off" It is completely tearing her apart! She is a very bright, beautiful, fun young lady. She takes blame for all of it. ?? She says that she is ruining our lives. She says, “I want to do well in school (which she always has) but I can’t lately, I am so tired.” “I can’t take it anymore Mom.” “I am so exhausted Mom, Please I just want sleep.” The pain and the lack of sleep have equally affected her life. As her Mom (and Dad) it is killing us!

Currently: Our daughter has been going to Bostons Children's Hospital for about 6 months now. She has been diagnosed with Pudendal Nerve Entrapment. Without known cause. (We can only guess). She is being seen at the Boston Children's Hospital Pain Clinic. She had a nerve block done in October. They said that she is the only child that they have ever done this on. She was pain free immediately. As the days followed she stayed on a pain scale of a 5-6. This lasted for about 1 and 3/4 months. Which according to everything I have read and her Doctor is pretty good. She also started to sleep on her own and was staying asleep (not waking up 3-4 times a night)!!!!! Yeah! But short lived. :( :( She has been in so much pain the past couple of weeks! It might be the worst that I have ever seen it! Poor baby! For a Mother (and Father) to see their child in that much pain and they can't do anything (except comfort them) is heart wrenching. This whole thing has literally broken our hearts!
One of the most aggravating aspects of this awful illness is that people will see her playing with her brother and friends, laughing, going to school, doing her homework, playing her Harp, playing soccer, watching t.v.......whatever it may be.......AND they will say, "She looks like she is fine now". "She doesn't look like she is in any pain." "She can't be in too much pain if she is doing that?" She/You have chronic pain, your not dead! I would be really worried if she DIDN'T do these things! When I had a chronic pain (neck injury,) I still got married, laughed, went to work, and had fun....but I did it all in pain! When you finally call it a day, then you might let go a little because nobody is around. I use to. The pain for me was sometimes worse at night. It is sometimes for my daughter too.
So...I talked to her Doctor in Boston last night.....we had a follow up appointment scheduled this Wednesday....but I couldn't see traveling several hundreds of miles to tell him what I could tell him over the phone. He has decided that he wants to try another nerve block. So she is scheduled for that this coming Thursday. We had a good conversation. He is interested in our daughter, which I appreciate! I understand that Nerve Blocks are only a temporary measure but have also dimensioned the pain a few. We really don't have many choices. She can't live her daily live like this!
We, for a long time thought my daughter was the only one! Little by little we are finding that there are many people suffering from the same awful disorder/illness! I haven't read everybody's story YET. But I will! We would really like to find a parent/parents out there with a young child that is suffering from the same symptoms as our daughter. Is there anybody???
Whew!
Thanks so much for your time!
I wish all of you well! HOPE for a cure!

You're the third mother of a young girl to visit the forums. The first found her daughter got a lot of relief from pelvic floor PT, the second is just getting more diagnostic information.

In my own case my pain didn't come on until after I gave birth. In surgery my two ligaments were found to be grown together, encasing the pudendal nerve like it was the lead in a wooden pencil. My surgeon said there's no way to tell if they grew together during my youth, or if I was just born that way. All I can figure out is that my nerve had just enough space in there for a long time, until one day it didn't. When nerves get compressed, they will swell to heal themselves. If they're entrapped by something that won't let them swell, they can grow new fibers and thicken, and sometimes push through the offending material. But they will send a constant pain signal.

I will say it is normal to be able to distract yourself and be able to do other things or just not think about pain for a while. Is anybody willing to give your daughter some medication to help her get better sleep? Even Neurontin at bedtime, for example? The sensation of chronic pain is only worsened by lack of sleep. I'm so sorry your family is going through this. I have a daughter and can only imagine the horror of having doctors think she was abused.

I'm not sure of which doctors may treat people under age 18 for PNE. I'm not sure how you would feel about surgery to address compression at her age. I think a lot would depend on if you felt 1000% sure you had the right diagnosis. Has she had an MRI? It might be worth a trip to see Hollis Potter in NYC for one; she is seeing a lot of PNE patients right now. It's strictly to help you feel like you have information about a diagnosis. She doesn't give treatment, and in fact the results of the MRI don't change your treatment options in any way. But it might help psychologically to feel like you have the right focus.

Best of luck to you with this.
PNE as a result of childbirth, 2002. Treatment by the Houston team, with neurosurgery by Dr. Ansell in 2004. My left side ST and SS ligaments were found to be grown together, encasing the pudendal nerve.

I am cured. I hope you will be, too.

There are no medical answers on the forum. Your only hope is to go to a doctor. I was very happy with the Houston team, which has treated the most PNE patients (well over 400), more than any other US provider.

I have heard a lot of good things re-guarding PT and PNE. Our daughter has gone to PT but really didn't get any results. Two visits we spent the whole time on their pain machine (reads your pain level). I believe that their are PNE PT specialists out there that I need to find. Yes, our daughter has had an MRI in Boston.
I have also heard a lot of good results with YOGI. Has anybody tried it?
Thank-you so much for taking the time to reply! I appreciate your knowledge and advice.

Oh, Tammy! I am so glad you found our site too! My heart goes out to you so much. I have had PN intensely for 8 years now... but probably had smaller symptoms as a girl (nothing like your girl... just constipation, painful bowel movements, leaning to one side when sitting, and tampons hurt in teens). But that small amount makes my heart go out to you. My PN developed after my first papsmere and when I got married. Within a year it went from just having vulvar pain and pain with intercourse to having pain while sitting. And it's stuck and I am seeking a cure. I know I will get there. But I understand her feelings of wanting her privates cut off, wanting to be in heaven, and trying to have fun while in pain. Been through all of that, and it's normal with this intensity of pain. I am so glad you got nerve blocks for her. Another mom on the forum (called MOM) has posted on "the journey of a child" her daughter's clitoral/labial itch that is progressing. It would be great to have you both in touch w/each other to empathize and encourage each other. I too have a daughter aged 4 and she has ouchy poopies and constipation... I fear for her. I am off to go buy some prunes and make her drink some water. I will write more later. If you PM me, I'd be happy to give you my number and talk on the phone about what I know. I've been to the following docs already (in order) Dr. Ken Renney, Dr. Aaron Filler (not recommended), PT Julie Sarton, Dr. Eric Bautrant, PT Stephanie Prendergast, Dr. Stanley Antolak, Dr. Michael Hibner. I've had nerve blocks with ALL of them except Prendergast. I can tell you a bit about each office. I hope to save people time through my journey. Will keep you in prayer! Thank God you are being proactive and taking your daughter's pain SERIOUSLY.
-straddle fall age 4-7 w/bleeding labia, tampons hurt in teens, papsmere started annoying pelvic 'tingling' & pne in 02
-obturator surgery w/ Filler in 05 (useless, created sciatic & plantar fascitis pain)
-TIR surgery w/ Bautrant in 08 and vestibulectomy in 08 in France (vest. removed pain w/intercourse, pain w/sitting increased post surgery)
-chronic fatigue & food allergies/migraines (gluten, milk) from pain meds in 08
-want a life back. I'm 34 w/8+ years of pain

Hi Tammy, I am glad you found us! I just want to give you the links to the 2 main threads by the other mother who is currently active on this board, with a 7 year old daughter:

http://www.pudendalhope.info/forum/view ... ?f=3&t=544

http://www.pudendalhope.info/forum/view ... ?f=3&t=601

They are kind of hard to locate and I think they might be useful to you. I am sorry your daughter is going through this.
pelvic pain started 1985 age 14 interstitial cystitis. Refused medical care from age 17, did GREAT with self care for years.
2004 PN started gradually, disabled by 2009. Underlying cause SIJD & Tarlov cysts
improved with PT & meds: neurontin, valium, nortriptyline, propanolol. (off nortriptyline & propanolol now, yay!)
Tarlov cyst surgery with Dr. Frank Feigenbaum March 20, 2012.
Results have been excellent so far; but I won't know my final functional level for a couple of years.

Tammy,
My heart is aching and tears are streaming. My duaghter is 7 and has been suffering since age 4 1/2
I am new to this journey but I really recomend you going to a PT who specializes in pelvic floor disfunction. Someone who is aware of PNE and is willing to do some research. Your daughter does NOT need to be in PT to see how much pain she is in you already know that she is in pain. Someone needs to have a clear picture of what her pelvic muscles are doing and work towards relieving that pressure on the nerve.
Please consult the specialist. I am goign to call Dr. Hibner this week for my daughter, maybe you should try him too. Dr's need to know children are suffering and we need to get them help. I have heard GREAT info about Dr. Hibner
I will PM you or you can me.
I am going tp prayer on your behalf and your daughters!
God comfort you

Dear Tammy,

My heart aches for you and your daughter and I hope you will find some answers here that will help her to live a pain-free life.

I can relate well to your comments about her difficulty with sleeping because I was very sleep deprived with this condition at one point. Can her doctors prescribe anything to help her sleep and relieve the pain at night? Is she currently on any medications? Medications made a huge difference for me and they were only temporary while I was finding the right treatments. I can honestly say that having medications to help me sleep made the difference between wanting to live and wanting to die. I am completely off all of them now. Your daughter's exhaustion and comments about not wanting to live suggest that her pain is not well-controlled. There is some trial and error in determining what works best for each person but it may be worth the effort.

One thing to remember about nerve blocks is that they have a potential risk -- some people have gotten worse from them and very few people are cured with them because they do not address the underlying cause. They are a valuable diagnostic tool as it appears they were in your daughter's case but beyond that, you may want to consider carefully whether you think they are the best treatment option. If you were to have your daughter see Dr. Hollis Potter in NYC for a 3T MRI and she determined that there was an entrapment, or a physical therapist determined there is a structural abnormality, all of the nerve blocks in the world will not solve the problem. So, I guess what I am trying to say is that nerve blocks have a bit of a bad rap among some PNE patients.

Tell your daughter we are all cheering her on and that there is a very good chance that with the right treatments she will get better and be able to live a normal life! Hang in there and be strong. ;)

Violet
PNE since 2002. Started from weightlifting. PNE surgery from Dr. Bautrant, Oct 2004. Pain now is usually a 0 and I can sit for hours on certain chairs. No longer take medication for PNE. Can work full time and do "The Firm" exercise program. 99% cured from PGAD. PNE surgery was right for me but it might not be for you. Do your research.

Thank-you Violet for your valuable information! I will definetly take all into consideration! I totally understand about the nerve blocks. When you are under one Doctors care, you want to trust them and know that they are leading you in the right direction. Although, Many times we have had to be the ones to give direction. Something that bothers me to no end, many of the Doctors that my daughter has seen, made us think/pretend that they know exactly what it is and how to cure her. So many of them have lead us in the wrong direction, given us bad advice, medication and results. We are very cautious with every medication, procedure and etc that our daughter receives....But when your daughter is in so much pain, you grasp at straws.....Example Nerve Block. Even a temporary fix is better than none. In the past few days I have learned more from this site and one other, then I have in the past 6 years! I think we are going to go for the 2nd nerve block (because the pain is so bad right now) and then go from there. There will not be anymore if the results of this one is the same as the last.
As far as sleep/pain medications.....we have had two bad experiences with medications that they have given our daughter. One put her in a very scary "box"/depression. Her doctor has said that there are others that he would like to try and we will do that after the nerve block. We are willing to give it another shot.
Also on our list of things to try....find a PT that knows about PNE. We would like to try Yoga. We also have received the name of a Doctor who specializes in pre-adolescent girls with Vulvodynia (out of state). A lot of things to think about.....guessing and second guessing.....
Again ....thank-you!
Best Wishes Always!

Thank-you so much for your reply and kind words. It means so much to us!
I wished we lived closer to your Doctors but we are across the country from you. We live in Maine.
We pray for you, our daughter and for all the people that are suffering from this awful "hidden" illness.

Mom
Thank-you for your reply. I have been looking for somebody, anybody going through what we are going through. Not that I would ever want somebody's child (or anybody) to ever go through this but it is nice to be understood. You are only the second mother that I have met that has a child going through what our daughter is going through. I just received an e-mail today from another mother with a 7 year old daughter with the same illness. Her story is more then heart breaking! I too had tears streaming down my face. It is is an unbelieveable story! I will send her to this site with hopes that she is willing to hook up with you also.
In my replies to other members that I have written in the past hour you will see what our next steps and hopes are. Never really sure if we are going in the right direction or doing the right thing. :(
God Bless you!
Lets talk soon!
