The thing is, bowel movements no longer cause me any discomfort since I started massaging my sphincter. When my bowel is full something is either being stretched or a trigger point is being pressed. I am trying to find out what is causing the tension. I have massaged the hell out of my sphincter muscles and it has reduced a lot of tension but not all. Its clear to me that I over stretched my sphincter muscles when I touched my toes, then I was sitting on them for long periods at work. This has caused them to spasm. I'll give the physio another 5 weeks to see if anything improves.

Hope the physio works for you reckless :)
Tummydepressed, take a look at the home pages where Violet has posted Maeve Whelans letter to HOPE she has some sample videos for pelvic floor release techniques, they are aimed at women but may work for men too reckless. They may be what you do already but worth a look.

Reckless,

Are you giving yourself regular warm sitz baths with epsom salts? Or having regular hot baths? I find that these relax my pelvic floor muscles tremendously. Also, sitting in a hot whirlpool tub.

Good luck with loosening up that pelvic floor. Sounds like you are on the right track.

I've been doing the stretches given by the pelvic physiotherapist. I haven't really noticed any improvent yet.

I've now booked a sports massage, I'm going to get the masseuse to focus on my hamstrings, abductors, groin and glutes.
Can this help relieve tension in the pelvic floor?

I just saw the sports massage therapist. He believes it's a nerve issue due to the fact I get tingling, burning and numbness. He said my pelvis is tilted to one side and I need to get it back into alignment. He's given me some stretches and did some massage.

He also said the pressure and tension that I feel in my crotch area is due to the nerves being impinged.
Does that sound right? Because I can't seem to pinpoint the exact area of tension.

Reckless, yes your massage therapists assessment makes a lot of sense. The only thing I would add is to be careful about stretching because if your nerve is entrapped you don't want to make it worse from a stretch injury. I believe that is what happened to me.

PNE since 2002. Started from weightlifting. PNE surgery from Dr. Bautrant, Oct 2004. Pain now is usually a 0 and I can sit for hours on certain chairs. No longer take medication for PNE. Can work full time and do "The Firm" exercise program. 99% cured from PGAD. PNE surgery was right for me but it might not be for you. Do your research.

I don't know what the cause is. The fact that it hurts a lot less when my bowels are full makes me think it is due to a tight pelvic floor. In my last couple of weeks at work I held off going to the bathroom for as long as possible and was able to sit with no pain. The full bowels must have been stretching the pelvic floor.
I've stopped doing this now as it's not good for you.

The hamstring stretch brings me a lot of relief when I am in pain. I was in a lot of pain 30 mins ago because I had to sit at the doctor's, I did the stretch and feel a lot better now.

I have two states that I am usually in.
1. No pain but a strong feeling of tension in the perineum. It's almost like an aroused feeling. But my orgasms don't feel right when I relieve myself and the feeling is still the same after.

2. Pain in my perineum and rectum which slowly moves to my penis given how long I am sitting. The feeling of tension goes away and is replaced with pain.

I need to get rid of the tension.

I need to find a way of getting rid of the tension.

I just had another physio appointment. The problem is clearly my sphincter muscle. It's too tight.
When I have a bowel movement I can feel that the stool is only coming out of the bottom end of the sphincter, the top part near the perineum is very tight, this also explains why my perineum is tight. I'm gonna buy a dilator to help stretch things out down there.

I've also got an appointment with a urologist coming up, is Botox helpful to relax the sphincter muscles?

Reckless, it's my understanding that Botox into the sphincter muscles can cause incontinence - you might want to do a bit more research before going that route. Some people have had botox into the puborectalis and coccygeus but you want to make sure the doc knows what they're doing if you try that because there is the potential for incontinence although it would most likely be temporary if the Botox wears off as it typically does in 3-6 months.

The anal sphincter is innervated by the pudendal nerve. What do you think might be the cause of the anal sphincter tightness?
