They were not guided blocks that i got. I just wonder why mine goes away on its own over time and stays away for 5-6 years. Very strange. And i dont hear to much on here about ppl having this awful sensitive feeling on sphincter. I am going to try PT i found on this site, she is about an hour and a half away from me. Thanks for your advise. I will keep posted on my treatment. Now its all about money.

Thanks for your advise, I just have pressure if i sit, stand, all the time. I will start with a PT when i can save up for it. I just hope that it goes away again. But i still need to find out why, and what makes it come back. Im sure its not PNE cause mine goes away. I just want answers, and a course to go down. I went and saw Dr. Weiss he does the non guided block through the vagina. Not sure what branch or if it was just the pudendal nerve. I have to save up money to get blocks done and i want to get an MRN if possible. Does anyone know anything about that and where to go for that?

SIJD can cause any knind of pudendal symptoms because it can cause impingement on the pudendal nerve directly or indirectly. One example is the fact that a misaligned / twisted pelvis often causes tension on the sacrotuberous and sacrospinous ligaments - the pudendal nerve runs between these so if they are pulled tight the nerve can become entrapped between them.
pelvic pain started 1985 age 14 interstitial cystitis. Refused medical care from age 17, did GREAT with self care for years.
2004 PN started gradually, disabled by 2009. Underlying cause SIJD & Tarlov cysts
improved with PT & meds: neurontin, valium, nortriptyline, propanolol. (off nortriptyline & propanolol now, yay!)
Tarlov cyst surgery with Dr. Frank Feigenbaum March 20, 2012.
Results have been excellent so far; but I won't know my final functional level for a couple of years.

After all you said PT is definitely in order to relax those pulling and tight muscles. Tell the PT what you said here so he or she can work those areasas well as having them do an evaluation and to either diagnose or point you in the right direction for next steps to fix and avoid future problems. It mayseem odd but the PTs actually know quit a bit about this condition and seem to be a very good place to start. My PT diagnosed PNE and then sent me to the PN doctor.

2007-08 pelvic muscles spasms treated by EGS. 6/27/10 sat too long on hard chair- spasms, EGS not work Botox help, cortisone shots in coccyx help, still pain, PT found PNE & sent me to Dr Marvel nerve blocks & MRN, TG left surgery 5/9/11. I have chronic bunion pain surgery at age 21. TG gave me back enough sitting to keep my job & join in some social activities. I wish the best to everyone! 2019 luck with orthotics from pedorthist & great PT allowing me to get off oxycodone.

Sunshine, you might be interested in an anatomy description from this book:

http://www.dartmouth.edu/~humananatomy/ ... _questions
http://tinyurl.com/8labjkc

External anal sphincter (sphincter ani externus).

The sphincter ani externus is supplied by the inferior rectal nerves and by a perineal branch of the fourth sacral nerve (S.N.4).

Maybe it's a problem with the nerves more proximal to the sacrum (4th sacral nerve) and that's why the pudendal nerve blocks didn't help at all -- and why sitting doesn't affect it. I don't know -- just brainstorming here.

There are variations in anatomy of the inferior rectal nerves.

Abstract
MATERIAL:

4 corpses (1 fresh, 3 formoled) without preliminary injection; age: 84 to 90.
METHOD:

a front access enabled us to show the origin of the inferior rectal nerve from the pudendal plexus, its course across the Alcook canal and the ischiorectal fossa. Then a posterior transgluteal access showed the short intragluteal course across the "biligamentary tunnel" (between sacrospinal and sacrotuberal ligaments) of nerves and vessels related to the anus, in particular the inconstant Morestin's nerve.
RESULTS AND CONCLUSION:

the compression of nerves causing perineal pain can occur at different places. Depending on the localisation of the origin and the course of the inferior rectal nerve (which change) in relation to the place of that compression, and also the existence or not of Morestin's nerve, the changing topography of these pains can be explained.

PNE since 2002. Started from weightlifting. PNE surgery from Dr. Bautrant, Oct 2004. Pain now is usually a 0 and I can sit for hours on certain chairs. No longer take medication for PNE. Can work full time and do "The Firm" exercise program. 99% cured from PGAD. PNE surgery was right for me but it might not be for you. Do your research.

thanks, im kinda leaning toward that since my sphincter is always sensative to clothing, and its 24/7 the muscle gets very sore. and its all rt there it doesnt move around its horrible, today i went to my sisters for dinner and we had to leave cause i was dying, all in the sphincter area. Do i see the same drs for something like that since it branches off the pudendal nerve? Holly shit, what a mess this is.

All the best luck to you. This is a mess of a condition and just trying to figure it out. I hope they can give you a clearer idea and diagnose of your particular situation as we are none two alike!

I hope so, since im paying out of pocket for it. Kinda hoping it would go away again sooner than later. Just dreaming i guess. I just wish my sphincter wasnt so tight all the time and seem to stop it from being like that. Has anyone gone to Houston? I would like to talk to someone who has been there and let me know what its like and how they do things. I dont know how im making everyday, I guess like i did every other time it has come, sad and feeling hopeless.
