Central Sensitization

Just wondering if others on here have had their doctors told them they have "Central Sensitization" (basically a wound up Central Nervous System generating more and more pain) and if so where all they experience symptoms.

For me, I have burning in the tops of my feet (especially when I am cold and I have major cold intolerance) and aching in the heels of my feet, sometimes I get tension headaches that can last several days/weeks, neck/shoulder muscles knots and pain, and pain over my hips, low back and sacrum. Sometimes these pains can be just as bad if not worse than the PN pain!
-11/08 vulvodynia began around conception of first & only pregnancy
-3/10 sacral/sitting pain began after SIJD manipulation
-Progressive widespread pain- central sensitization
-PT, meds, injections, botox, ESWT = debilitated.
-5/12 Potter MRI - scarring of left ST, coccygeous & posterior alcock
-12/12 - left FAI/labral hip tear surgery
2014-2019 managed w/ gabapentin, massage, and lifestyle mod
2020 - big flare up

I am suspicious of this diagnosis for you Faith because you have a diagnosed pelvic misalignment: The pelvis is the kinetic center of the body and having a joint dysfunction there will misalign you from head to toe.
My 1st pelvic injury was when I was 14 and I got interstitial cystitis as a result and at the same time began to get migraines as my neck locked up in compensation for my pelvis. Of course nobody at the time knew that intertitial cystitis had a biomechanical component, so I was told it was probably an autoimmune disorder and I never knew my pelvis was injured.
I did not know my neck was locked up until I was chatting with a Chiropractor in my 20's...I mentioned my headaches and he checked my neck...now mind you I could barely turn my head but this was my "normal" so I really did not think much of it...The Chiro said my neck was so bad I must have been born with forceps or been in a car wreck. I told him neither was true. He adjusted my neck which became temporarily VERY loose by my prior standards...could turn my head so far it made me dizzy...but what I did not know at the time and the Chiro failed to notice, was that my neck was compensating for the pelvis. So, when he freed up the neck, my pelvis began to compensate elsewhere and I got low back pain for the 1st time in my life. So, I declined to see the Chiro anymore and gradually I returned to "my normal" - a comfy back and a stiff neck.
When my pelvis was finally examined properly by Jerry Hesch, he demonstrated to me prior to adjusting the pelvis, that I actually had joints locked up from my jaw to my ankles, secondary to the pelvic dysfunction. When he adjusted the pelvis, the other locked-up areas such as jaw, neck, sternum, etc released spontaneously without treatment.
I have neuropathy of the fingers that I got 7 or 8 years prior to the PN (even before 2nd pelvic injury in childbirth), and got neuropathy in the toes and feet prior to PN after the childbirth injury (symptoms came on slowly...feet, the pubic, then anal, then neurogenic bladder) I have had migraine syndrome since the 1st pelvic injury including migraine headaches, optical migraines, vertigo, and a small clot in my brain. I have 16 of the 18 "tender points" of fibromyalgia although I in no way qualify for the diagnosis as I do not have musculoskeletal pain and fatigue.
But you have to realize: When your pelvis is twisted, if only the pelvis were twisted you would be walking kind of diagonally or lopsided,, and your body will not let that happen. It had powerful reflexes to keep your eyes level and the circulation to your brain even on both sides, and if your pelvis is twisted your body is going to twist someplace else to make that happen. So whole-body symptoms with a pelvic joint abnormality are the norm because of the rules of biomechanics / kinesiology - this is not "central sensitization"...but MD's are NOT kinesiologists or biomechanical experts, so that would be their best guess I suppose.
pelvic pain started 1985 age 14 interstitial cystitis. Refused medical care from age 17, did GREAT with self care for years.
2004 PN started gradually, disabled by 2009. Underlying cause SIJD & Tarlov cysts
improved with PT & meds: neurontin, valium, nortriptyline, propanolol. (off nortriptyline & propanolol now, yay!)
Tarlov cyst surgery with Dr. Frank Feigenbaum March 20, 2012.
Results have been excellent so far; but I won't know my final functional level for a couple of years.

What type of doctor can check and tell you if your pelvis is out of alignment? I've started paying real close attention to my body and think I'm walking lopsided. When I get tired I actually start to drag my left leg. This also may be part of the reason the right ankle swells, it's compensating. Hmmmmmm.
Have been dealing with burning pain since Jan 2010.
No sitting since April 2010.
Seen the following dr's: DO, GYN, Dermatologist, Accupuncturist,
URO GYN (his RN is the one who suggested the pain could be PN), Neurologist
Had ECG and MRI both inconclusive, only the SSEP said Pudendal reaction was abnormal and they lost that test result.
Saw Dr. Castellanos April 6, 2011. Next steps, MRI and botox. Having PT while waiting.-Botox denied, appealing to Insurance company now. :(

Tracy: PT Manual Therapist. It is a small subset of PT practice. Some PT's in various specialties especially pelvic floor or sports PT have also pursued training in pelvic alignment, but this is very individual and you would have to enquire.
