New comer with a few questions and weird history

Post by Acschiro »

I had a vasectomy on 4/5 and developed a large hematoma develop I couldn't stand up. On the 5th day I went to get out of bed and felt sharp electrical shock from left ischium to scrotum. Assumed it was due to vasectomy. Since than I have had only 2 weeks where I could sit more than 5 minutes. To make a long story short my presnt symptoms are testicle pain bilateral with radiation along genitofemoral nerve, my anus has electrical prickly feeling, deep dull pelvic floor pain was originally along transverse perineal muscle with tp's now mostly left lavator ani, siting on toilet relieves lying down does not. I have had numbness bilateral scrotum, but cheeks and posterior thighs. I can no longer wear boxer briefs they feel itchy and irritable, severe pain behind testicles with sex and painful erection, was also diagnosed with a prostate infection which has cleared. Saw post vasectomy pain doc and had genitofemorql blocks yesterday. He thinks it triggered all the nerves, but I'm a little doubtful the blocks helped testicles, but not pudendal region. I have a long history of a bad left hip socket which was negative on MRI and a really bad left SI joint. Talked to Dr. Weiss on phone he said the vasectomy could have been the perfect storm to finally allow the bad left hip and si joint to cause the pudendal due to muscle guarding. But I told him I get relief with squatting and he said it should make it worse? I've seen a pelvic floor specialist but she's the only one in 100 miles and has only had a couple of cases of pudendal nerve. Has anyone seen Dr. Weiss. I am a chiropractor so I understand the anatomy and we both kind of think it is more of a muscle irritating nerve. I also have bilateral Morton's big toes and completely flat feet. If you've seen dr Weiss could you let me know if you liked the office. Was wanting some input before I fly across country.

Welcome to the forum. Sorry to hear of this pain you've developed.

I've not seen Dr. Weiss personally but have read reports from others who did. My understanding is that he does unguided nerve blocks using his finger to localize the pain and then delivers the pudendal nerve block there. He may also do some myofascial release and trigger point injections -- can't remember for sure. Also some people reported that they had a series of numerous nerve blocks from him with not much improvement (more than the 3 that are typically recommended). Last I heard he was in his 70's and that was a few years back. Unfortunately, the forum where I read these accounts has shut down. You might want to contact Stephanie Prendergast and Liz Rummer because I think they used to work with him (although I could be wrong). They are in the SF bay area and they are on our list of PT's.

Personally I am not a big fan of numerous nerve blocks because in my 10 years of reading/moderating the forums I just haven't heard of many people who got better from them and I do know people who had long-term worsening of symptoms from numerous nerve blocks. Are you planning to see Dr. Weiss primarily for a nerve block? Not sure where you live but if you are flying across the country to see him, might it be possible to see someone closer to you and get an image guided nerve block?

When I had PNE, squatting did not make my symptoms worse at the moment I was squatting. At the moment, the squats helped relax the pelvic floor but I believe over time, the squats that my PT had me doing caused a stretch injury to the nerve and sent me into full-blown PN due to a ligament entrapment. I also had SI joint issues with pelvic misalignment/nerve impingement/pain on the R side. Hopefully, with some myofascial release, you can get things to calm down but it sounds like you've got a bunch of musculoskeletal issues going on there. Did the MRI check for femoral acetabular labral tears? Do you know why your SI joint is bad? Mine was bad because my SI ligaments were hardened and sclerosed (discovered during surgery) and due to weightlifting they became chronically strained so my pelvis became hypermobile. This caused impingement on the nerve. It's really difficult to determine when the nerve is entrapped or whether it's just a muscle tension issue, so starting with a knowledgeable PT is typically considered the safe, conservative route to take as far as treatment. Hope yours will be able to help you.

Best,

Violet
PNE since 2002. Started from weightlifting. PNE surgery from Dr. Bautrant, Oct 2004. Pain now is usually a 0 and I can sit for hours on certain chairs. No longer take medication for PNE. Can work full time and do "The Firm" exercise program. 99% cured from PGAD. PNE surgery was right for me but it might not be for you. Do your research.

The Si joint is hypermobile from my job. I was looking at dr Weiss cause I heard he looks at pelvic biomechanics and feet. Not interested in blocks yet since we aren't sure if its vasectomy related or pudendal from si joint. There are no good pts anywhere near here closest is 1:30 and I can't see her till November. Thanks

The squatting also seems to set my SI joint. I'll have to check MRI to see if it looked for tears
Thanks

Hi Aschiro

Sorry about the pain you are in. It's all a lot to deal with. Wanted to say that my SI joint is definitely an issue and I've been told other joints are hypermobile. Wanted to say that on the way home today I was thinking back to an original theory I had that my knee (which was hurt in a ski accident and moves laterally when it shouldn't) throws everything off balance. I also have VERY flat feet. I've always wondered if something mechanical like that could be causing all this but wasn't sure how to pursue it as one doc said he could put me in two knee braces and two ankle braces if I wanted him too, but otherwise he couldn't help me. He was a gem.

I've been told sacrum belts to stabilize the sacrum can be helpful. They were just too tight around my bladder for comfort.

You could check with Jerry Hesch to see if there is anyone near you who uses the Hesch method of manual therapy for pelvic biomechanics. Here's his website: http://www.heschinstitute.com/
Or you could send Hermajesty a private message. She could give you more info on the Hesch method.

Thanks I really believe the pudendal is being triggered by faulty SI joint, but I think it is due to hip socket. Walking today I could tell socket wasn't gliding anteriorly and stuck in adduction and internal rotation. I forced it into abduction and external rotation and SI joint slid back in again. I'll check out that website and gonna call my MD friend and get MRI pelvis and arthrogrAm left hip socket my previous MRI of left hip was standard and didn't show anything but I looked at it and it was a little blurry. My ortho friend does not sclerosis on superior pole of femur head from xrays I shot on myself at office. Guess its time to get all my ducks in a row and tell my MD friends what tests they are doing instead of just drugging me up!
Again thanks for your help
