Pudendal Neuralgia. Pain Management Tactics

Post by jrxjoe@gmail.com »

Is there anyone willing to share their tactics on dealing with pain.
If there is, I would like to share my tactics and hopefully discover some new ones

Hmmmm....tactics for pain relief? Good question. I used a lot of gel ice packs, heating pads, and alternating hot/cold sitz baths. I described the sitz baths here: viewtopic.php?t=5267

I also used benzocaine ointment (extra strength vagisil - the walmart brand) but I've heard Premjact spray can help guys. Sometimes I used capsaicin (soothenol brand).

I used lexapro, an SSRI antidepressant for urinary discomfort. A lot of people use tricyclic antidepressants for urinary symptoms. For pain I took oxycodone in the evening/bedtime so I could sleep but I didn't take it around the clock because I didn't want to become addicted or develop a tolerance. It's harder to get oxycodone anymore though. Some people have had good results with buprenorphine because it's less likely to cause an addiction but it can be difficult to find a provider who is willing and who knows how to prescribe it properly. It's probably easier to get a prescription for something like lyrica and cymbalta.

I would be interested in hearing what works for you.

PNE since 2002. Started from weightlifting. PNE surgery from Dr. Bautrant, Oct 2004. Pain now is usually a 0 and I can sit for hours on certain chairs. No longer take medication for PNE. Can work full time and do "The Firm" exercise program. 99% cured from PGAD. PNE surgery was right for me but it might not be for you. Do your research.

Hi jrxjoe,

I used both ice and heat. I used ice almost constantly for a couple of years (I put water in condoms, froze them, wrapped them in a wet paper towel, and placed them on a pad in my underwear so they would freeze up my pelvic floor). I eventually developed severe panniculitis from that. Then I discovered that high heat (heating pad on high) worked. The heating pad is safer for long term use, but, of course, it can only be used at home. I was also on several medications (e.g., nortriptyline, amitriptyline, gabapentin, tramadol) that also helped, I think. I tried pt and nerve blocks but neither of those helped. Now that my pain is so much lower, I just use a heating pad and only need it periodically.
